Update: CALS Advancement / Ledger 7 Deposit Form

The University Advancement office recently updated the “Notification of Gift Transmittal,” also known as the BA-151 form. Please use this link to access the form directly.

The BA-151 form has also been updated on the CBO Documents & Forms webpage, and you can use the Advancement Forms Directory to access all Advancement forms.

CALS departments should use this updated form in order to be consistent with university documentation. Starting April 1, 2026, all Ledger 7 deposits will need to use the university updated form.
