Completion and Routing of Vendor Forms

Please note that there are two types of Vendor Forms. The first is the standard form that requires completion and signature by NC State to allow NC State to become a vendor within a Sponsor’s system. The second is an electronic registration that creates NC State within a Sponsor’s electronic vendor system. 

Per the University Controller’s Office, any electronic registration requests to set up NC State as a vendor should go through the Cash Services Office so that we aren’t duplicating information. Examples are PaymentWorks or PaymodeX registration requests. 

For a non-electronic/standard vendor form, the CALS department should complete the fields, as applicable, and then route the editable form through ServiceNow via the “Get Help” request. In the request, you can select “Other” for the Category and indicate in the “Please describe your issue” field that you are routing a Vendor Form for review and signature by CBO A/R. Please attach the editable form to the requestOnce received, the ServiceNow incident will then be redirected to CBO A/R.
