Controller’s Office Travel Forms Updated

Earlier this year, the University implemented specific requirements for signatures. To ensure compliance with the updated policy, the Controller’s Office updated the electronic signature verbiage on all official Controller’s Office forms. The old Travel Forms (e.g., AP104 Non-Employee Travel Authorization) will no longer be accepted by the Controller’s Office as of November 1. The updated forms will display “Updated 10/2024” in the upper right-hand corner for easy identification. 

Please be sure you are now using the most up-to-date Travel Forms. The links on the CBO Documents & Forms page are now updated.
