This form replaces the PDF version of the &#8220;Alcohol Permission Form&#8221; on the CBO <a href=\"https:\/\/units.cals.ncsu.edu\/core-support\/documents-and-forms\/#business-services\" target=\"_blank\" rel=\"noreferrer noopener\">Documents &amp; Forms<\/a> page. CALS units should use this form when requesting permission to serve alcoholic beverages at a university event, and once submitted, the form will automatically route to the Dean for approval.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This new process supports a more efficient electronic approval workflow and helps ensure compliance with university requirements.
